How do I upload my ID?
To upload your ID you can navigate to the "Home" page of the Patient Portal. Once there, you can upload your ID by going through the following steps:
1.) You will likely have a notification on the main page of the Patient Portal, directing you to complete ID verification. This notification will also appear under the bell-shaped notification icon in the top right of the Patient Portal.
2.) Either click the arrow on the homepage, or the "GO" button in the bell notification where you will begin the ID verification process
3.) Select which type of ID you would like to upload
4.) Follow the on-screen instructions that vary, depending on the ID you have selected.
5.) Allow the ID verification to take place
6.) Verify that you have completed the process
