How do I complete my intake?
To access your intake (if it has not been filled out already) you can navigate to the "Home" page of the Patient Portal. Once there, you can complete your intake by going through the following steps:
1.) You will likely have a notification on your main page of the Patient Portal, directing you to complete the intake form. This notification will also appear under the bell-shaped notification icon in the top right of the Patient Portal.
2.) Either click the arrow on the homepage, or the "GO" button in the bell notification and then complete your intake.
Please Note: You may see a notification telling you that an intake needs to be completed. If you have already completed your intake please disregard the notification.
